What to Bring to Your Appointment
- Doctor's Prescription for Therapy.
- Insurance Card and Driver's License.
- For your convenience and to save time, you can download your
Medical History Intake Form and Admission and Financial Form here.
Please arrive 15 – 20 minutes prior to your first appointment to complete clinic and insurance papers.
- Bring shorts and tee shirt.
All Co-pays, co-insurance and/or deductibles are due at time of visit. For your convenience, we accept cash, personal check, MasterCard, Visa and Discover.
